  • Preventive care: cleanings and exams, generally covered at 100% with no copay.
  • Basic care: fillings, deep gum cleanings, and simple extractions, shared between the plan and the patient.
  • Major care: crowns, bridges, dentures, and oral surgery — requiring plan approval and higher patient coinsurance.

Many plans also provide an annual dental dollar maximum, or a reloadable health spending card that gives you a dedicated allowance for dental visits each year.

Prior Authorization: What to Expect Before Major Work

A prior authorization (PA) is an advance review: our dental team sends your proposed treatment plan, X-rays, and clinical notes to your Medicare Advantage plan for approval before major work starts. It exists to protect you — it confirms the plan agrees the treatment is necessary, checks your tooth against the plan’s guidelines, and sets out exactly what the insurance will pay, so no unexpected bill arrives later.

Routine cleanings, regular exams, cavity-check X-rays, and emergency visits for pain relief never require prior authorization. Major treatment almost always does:

  • New crowns
  • Foundation core buildups
  • Back tooth (molar) root canals
  • Deep gum cleanings
  • Full or partial dentures
  • Denture relines
  • Surgical extractions
  • Jawbone smoothing (alveoloplasty)

We handle the whole submission for you — up-to-date digital X-rays showing the full tooth and root tip, a gentle gum-measuring check (periodontal chart), and a written clinical explanation of why the tooth needs that care. Plans take 7 to 14 business days to return an approval voucher; urgent emergency requests can be decided in as little as 72 hours. Dual-Eligible Plans (D-SNP) in South Carolina

If you have both Medicare and South Carolina Healthy Connections Medicaid, you qualify for a Dual Eligible Special Needs Plan such as First Choice VIP Care, Humana Dual Plus, or UnitedHealthcare Dual Complete. These plans coordinate your medical and dental benefits under one card.

Medicare pays first and South Carolina Medicaid pays second, so covered dental procedures carry no out-of-pocket copayments or deductibles. Under federal law, our office is forbidden from balance-billing dual-eligible patients for any covered service.

Many South Carolina D-SNP plans also include a pre-loaded healthcare spending card — a UCard or a Humana Spending Account Card, for example — with an allowance between $1,500 and $4,000 per year that can be used directly for dental treatment.

Plan Guidelines, Annual Limits & Exclusions

A few practical rules decide how far your Medicare dental benefit goes:

  • Annual dollar maximum: most standard Medicare Advantage plans cap the benefit between $1,000 and $3,000 per calendar year. Care beyond the cap is your responsibility at discounted network rates.
  • Deductibles and coinsurance: cleanings and checkups have no deductible. Basic fillings and major crowns may carry a small annual deductible of $50 to $100 before cost-sharing begins.
  • Replacement waiting clocks: crowns and complete dentures are usually covered once every 5 years (60 rolling months). A denture fitted three years ago will not be replaced until the full five years have passed.
  • Missing tooth rule: some private plans will not pay to replace a tooth with a bridge or partial denture if that tooth was pulled before you joined that specific plan.
  • Common exclusions: routine Medicare Advantage dental riders do not cover purely cosmetic treatment such as teeth whitening, porcelain veneers, or sports mouthguards. Dental implants are covered only if your plan includes a specific upgraded implant rider.
