These signs point to infection or trauma that generally worsens without professional treatment.
Facial swelling suggests the infection has spread beyond the tooth into surrounding tissue, and it should be assessed promptly. Swelling that reaches the eye or the floor of the mouth, or that makes breathing or swallowing difficult, is a medical emergency needing immediate hospital care rather than a routine dental appointment.
Pick the tooth up by the crown, never the root, and rinse it briefly with milk or saline if it is dirty. If possible, place it back into the socket and bite gently on a clean cloth. Otherwise keep it in milk or saliva and get to a dentist as fast as you can.
Waiting is risky when infection or a fracture is involved, because both tend to progress. A small area of decay can reach the nerve, and a contained infection can spread into surrounding tissue and become far harder to treat. Pain that fades on its own is not reassuring, since it can signal nerve death.
